Preparing for your first visit to Turkey Creek Dental is easy. To ensure a seamless experience, please bring the following items with you:

  • Your Dental Insurance Card: Turkey Creek Dental is committed to providing exceptional care to everyone. As we’ve recently updated our network of accepted insurance carriers, we encourage you to call us in advance to confirm whether your insurance is in-network. 
  • Identification: Please bring a valid form of identification, such as a driver’s license or state ID, to your appointment. 
  • Patient Forms: To save time, we’ve made new patient forms available on our website.  Please bring completed forms to your appointment or arrive 15 minutes early to fill them out at our office.
  • List of Medications: If you’re currently taking any medications, having a list on hand will assist our dental team in providing you with the safest and most effective care tailored to your needs.
